Why Do Wisdom Teeth Cause Pain?
Most people do not have sufficient jaw space for their four wisdom teeth (third molars). When they attempt to erupt, several problems can occur:
Impaction: The tooth cannot fully emerge and presses sideways into the neighbouring molar or bone. This causes persistent aching pain, often worse at night.

Ibuprofen (400mg) with food every 6–8 hours is the most effective over-the-counter option — it reduces both pain and inflammation.
Clove oil: Apply a tiny amount directly to the gum over the painful tooth with a cotton ball. Provides 30–60 minutes of temporary relief due to the natural anaesthetic compound eugenol.
Salt water rinse: Half teaspoon of salt in warm water, rinse gently 3–4 times daily to reduce bacterial load and inflammation.
Cold compress: 15 minutes on, 15 minutes off against the outside of the cheek to reduce swelling.
These measures provide temporary relief only. They do not treat the underlying problem.
Does a Wisdom Tooth Always Need Extraction?
No — but most impacted wisdom teeth do eventually cause problems. Extract promptly if you have had more than one infection episode, there is decay in the wisdom tooth or adjacent molar, a cyst is visible on X-ray, or you are about to start orthodontic treatment.
Monitor conservatively if the tooth is fully erupted in good position and you can clean it adequately.
